Primary Care

Medical care where residents live.

Our licensed providers support residents’ everyday and ongoing medical needs within participating assisted living communities. Care is guided by the resident’s needs, with follow-up and coordination as appropriate.

Chronic Care Management

Support between visits.

Living with chronic conditions can involve ongoing monitoring, follow-up and care from several providers. SGC CARE offers chronic care management for eligible residents, helping connect these needs through continued support and coordination. Our team can explain program eligibility, what participation involves and any costs that may apply.

Care Delivery

In person and through telehealth.

Providers see residents within participating communities. Telehealth may also be used when appropriate for the resident, the service and the clinical situation.

Care Coordination

Keeping care connected.

We coordinate with community staff, other healthcare providers and authorized family members as appropriate, helping support continuity as residents’ needs change.

Let’s discuss the care you need.

Service availability depends on location, community participation, clinical need and eligibility.
